Enalapril maleate
Indications/Uses
Treatment of HTN & heart failure. Prevention of signs of heart failure.
Dosage/Direction for Use
Adult HTN Initially 5-20 mg once daily. Max long-term dose of 40 mg once daily. Heart failure Initially 2.5 mg once daily, may be increased to 20 mg in once daily or bd as long-term dose. Max long-term of 40 mg bd. Kidney problem: Moderate 5-10 mg/day. Severe 2.5 mg/day. Childn >50 kg 5 mg/day. Max of 40 mg/day. 20-50 kg 2.5 mg/day. Max of 20 mg/day.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to enalapril maleate or other ACE inhibitors; known or history of allergy or angioedema. Concomitant use w/ aliskiren-containing products in patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m2). Pregnancy (2nd or 3rd trimesters).
Special Precautions
Heart problem, conditions involving blood vessels in brain; neutropenia, thrombocytopenia, anaemia; liver problem; kidney problem including kidney transplant, patient having dialysis, excessive vomiting or diarrhoea, salt-restricted diet or taking K supplements, K-sparing agents or K-containing salt substitutes, hypotension, collagen vascular diseases (eg, lupus erythematosus, rheumatoid arthritis or scleroderma). Monitor K & blood glucose levels. Surgery, concomitant use w/ anaesth; LDL apheresis; desensitisation treatment. Dual blockade of the renin-angiotensin-aldosterone system (RAAS) through the combined use of ACE inhibitors, angiotensin II receptor blockers or aliskiren is not recommended. May impair ability to drive or operate machinery. Black patient. Lactation. Elderly >70 yr.
Adverse Reactions
Dizziness, weakness, blurred vision, cough; hypotension, bradycardia/tachycardia, chest pain, headache, syncope (fainting), taste alterations, shortness of breath, diarrhoea, abdominal pain, rash; swelling of face, lips or tongue; fatigue, depression; increased creatinine & K levels.
Drug Interactions
β-blockers, diuretics, K-containing agents, antidiabetics, lithium, TCAs, antipsychotics, sympathomimetic agents, NSAIDs including COX-2 inhibitors, aspirin & other thrombolytics, alcohol. Higher frequency of adverse events w/ combined use of ACE inhibitors, angiotensin II receptor blockers, or aliskiren.
